You’ve tried the expensive face washes. You’ve tried the dermatologist-recommended creams. You’ve cut dairy, tried niacinamide serums, and spent half your salary at the skincare aisle. And yet the acne keeps coming back, or the eczema won’t stop flaring up, or your skin just looks perpetually dull and irritated.

What if the problem isn’t on your skin at all? What if it starts about five feet lower, in your gut?

The gut-skin axis is a real, well-documented biological pathway. And understanding it might save you a lot of money on products that only treat the surface.

The idea that the gut influences the skin isn’t new. Dermatologists John Stokes and Donald Pillsbury proposed a gut-brain-skin connection back in 1930. They hypothesised that emotional states alter gut flora, increase intestinal permeability, and contribute to skin inflammation. Nearly a century later, modern research has proven them remarkably right.

The basic mechanism goes like this. When your gut microbiome is out of balance (fewer beneficial species, more inflammatory ones) the lining of your intestine can become more permeable than it should be. This is sometimes called "leaky gut," and while the term gets overused on wellness Instagram, the underlying science is solid. When those tight junctions between intestinal cells loosen, bacterial fragments like lipopolysaccharides (LPS) and other inflammatory molecules slip into your bloodstream.

Your immune system treats these molecules as threats and mounts an inflammatory response. The problem is that this inflammation doesn’t stay local. It goes systemic, circulating through your blood and showing up in your skin as acne, eczema flare-ups, rosacea, or psoriasis.

Your skin ends up being a billboard for what’s happening inside. When internal inflammation is high, your skin is often the first place it shows up.

If you’re dealing with persistent acne, especially the deep, cystic kind that keeps returning no matter what topical treatment you use, your gut might be worth investigating.

A study published in Gut Pathogens found that 54% of acne patients have significantly altered gut microbiota compared to people with clear skin. Specifically, acne patients tend to have lower populations of Lactobacillus and Bifidobacterium species, the same bacteria that help maintain gut barrier integrity and modulate immune responses.

The connection works through several pathways. First, gut dysbiosis increases systemic inflammation, which raises levels of insulin-like growth factor 1 (IGF-1) and androgens, both of which increase sebum production. More sebum means more fuel for Cutibacterium acnes, the bacterium that thrives in clogged pores and drives acne lesions.

Second, gut-derived inflammation activates a molecule called substance P, which directly stimulates sebaceous glands in the skin. So your gut bacteria can literally be telling your skin to produce more oil.

Most dermatologists in India won’t mention this: the standard Indian diet shift (more processed foods, less fibre, more refined sugar) is terrible for acne not because of some vague "toxin" story, but because it starves beneficial gut bacteria and promotes the inflammatory species. The teenager drinking chai with four spoons of sugar and eating Maggi for every other meal isn’t just eating junk: they’re reshaping their gut microbiome in ways that show up on their face.

The connection works through several pathways.

Eczema, psoriasis, and the immune connection

Eczema (atopic dermatitis) is fundamentally an immune disorder, and the gut’s role in immune training makes the microbiome connection especially strong here.

Research has shown that infants who develop eczema tend to have less diverse gut microbiomes in the first few months of life, with notably lower levels of Bifidobacterium species. This isn’t a coincidence, these early-life bacteria help calibrate the developing immune system. Without adequate microbial diversity, the immune system is more likely to overreact to harmless environmental triggers, which is exactly what happens in eczema.

In adults, the pattern continues. A 2019 study in The Journal of Allergy and Clinical Immunology found that adults with eczema had distinct gut microbial profiles compared to healthy controls, with higher levels of Staphylococcus aureus, a bacterium associated with skin barrier dysfunction and inflammation.

Psoriasis tells a similar story but through a different immune pathway. Psoriasis is driven by overactive Th17 immune cells, which produce inflammatory cytokines like IL-17. Gut bacteria directly influence Th17 cell development. When the gut microbiome is skewed towards inflammatory species, Th17 activity increases and psoriatic plaques can worsen. Several studies have found that psoriasis patients have reduced levels of Faecalibacterium prausnitzii, one of the most important anti-inflammatory gut bacteria.

Rosacea fits the pattern too. There’s a strong association between rosacea and small intestinal bacterial overgrowth (SIBO). A study in the Journal of the American Academy of Dermatology found that rosacea patients were significantly more likely to have SIBO, and treating the SIBO led to improvement or complete resolution of rosacea symptoms in many cases.

What this means for your skincare routine

None of this means you should throw away your sunscreen and cleansers. Topical skincare has its place. But if you’re dealing with chronic, recurring skin issues that don’t respond to surface-level treatments, the problem might be deeper than your epidermis.

The gut-skin axis suggests a different approach: instead of only treating the symptoms visible on your skin, also address the inflammatory environment that your gut might be creating internally. This means paying attention to dietary fibre (your gut bacteria’s preferred food), fermented foods like homemade dahi and traditionally prepared pickles (which contribute beneficial microbes), and reducing the processed food intake that feeds inflammatory species.

It also means that broad-spectrum antibiotics (often prescribed for severe acne) might be solving one problem while creating another. These antibiotics kill acne-causing bacteria on the skin but also devastate beneficial gut bacteria, potentially setting up a cycle of recurring skin problems.
