Why Most Probiotics Don't Work, and Sometimes Make Things Worse

You’ve seen them. Lined up at the pharmacy counter, popping up in Instagram ads. Capsules promising better digestion, clearer skin, more energy, weight loss, basically everything short of a promotion. So you buy a bottle, take it for a few weeks, and… nothing. Or worse, you feel more bloated than before.

When people say "probiotic," they mean any supplement with live bacteria. That’s like saying "medicine" without naming one. You wouldn’t take a diabetes drug for a headache. Same logic with bacteria.

Every probiotic contains specific strains, and each strain does something different. Lactobacillus rhamnosus GG is well studied for preventing antibiotic-associated diarrhoea. Bifidobacterium longum has helped IBS symptoms in some trials. Saccharomyces boulardii (a yeast, not a bacterium) can help with traveller’s diarrhoea.

Most commercial probiotics won’t tell you the exact strain. The label says "Lactobacillus acidophilus" and stops there. But there are hundreds of sub-strains of L. acidophilus, and they don’t all behave the same. Some make anti-inflammatory compounds. Some don’t. Some settle in your gut briefly. Some pass straight through doing nothing.

So when you grab a random probiotic off a shelf, you’re guessing. And guessing with your microbiome (a system more complex than your brain’s neural network) rarely pays off.

Say you pick the right strain. There’s still the question of how many bacteria reach your gut alive.

Your stomach is an acid bath, pH 1.5 to 3.5. Most bacteria don’t survive it. Studies show many commercial probiotics lose 90-99% of their viable organisms passing through stomach acid and bile. That "10 billion CFU" on the label? You might get 100 million live bacteria to your intestines. Sounds like plenty, until you remember your gut already holds roughly 38 trillion microorganisms. It’s a cup of water tipped into the Ganga, expecting the river to change colour.

Good probiotics use enteric coatings, microencapsulation, or spore-forming bacteria that resist acid naturally. Those cost money. Most Rs 200-500 bottles at the local chemist don’t bother.

There’s still the question of how many bacteria reach your gut alive.

When probiotics actually make things worse

This part rarely comes up. For some people, probiotics don’t just fail. They harm.

If you have Small Intestinal Bacterial Overgrowth (SIBO), adding more bacteria is like pouring ghee on a fire. SIBO is when bacteria that belong in your large intestine creep up into the small intestine. A probiotic (especially with Lactobacillus strains) can feed that overgrowth and make bloating, gas, and pain noticeably worse.

Research in Clinical and Translational Gastroenterology found probiotic use linked to brain fog and severe bloating in patients with bacterial overgrowth in the small intestine. Stop the probiotics and their symptoms improved.

Same logic if your gut already has too much of one species. Add more through a supplement and you create imbalance. Picture an office where the marketing team is already too big and someone keeps hiring marketers. Engineering and finance (your other good bacteria) get drowned out.

India’s wild west supplement market

In India the problem is sharper. The probiotic market here is largely unregulated next to the US or EU. FSSAI has guidelines, but enforcement is patchy. A 2019 study of commercial probiotics sold in India found many products didn’t contain the strains on their labels. Some had fewer live organisms than claimed. A few held contaminant species that shouldn’t have been there at all.

Stack strain mismatch, poor survivability, and shaky manufacturing, and the average Indian buyer is playing bacterial roulette.

What actually matters

The science is clear on one point. Probiotics work best when matched to your specific gut. A 2018 study from the Weizmann Institute of Science showed people’s guts responded very differently to the same probiotic. Some microbiomes took in the new bacteria. Others rejected them outright. The researchers called the latter "resisters," whose existing community essentially blocked the incoming strains.

Your microbiome is as unique as a fingerprint. It was shaped by where you were born, whether you arrived vaginally or by C-section, what you ate as a child, your antibiotic history, your stress, even the water you drink. A single capsule built for a generic "healthy adult" accounts for none of that.

This doesn’t mean every probiotic is useless. It means grabbing whatever’s on sale is flawed from the start. Effective use means knowing what’s already in your gut, what’s missing, and which strains might actually help your situation.
