The One-Size-Fits-All Problem With Probiotic Supplements

Picture a shoe store that stocks one size. Size 8. Doesn’t matter if you’re a 6 or an 11, you get an 8. That’s basically the probiotic industry. One formula, billions of customers, wildly different guts.

And we keep buying. Indians spent over Rs 1,500 crore on probiotic supplements last year. Most of those products carry the same handful of strains at roughly the same doses. They assume your gut looks like everyone else’s. It doesn’t.

Two healthy adults standing side by side in a Mumbai local can share as little as 10-20% of the same bacterial species. Read that again. Two perfectly healthy people, and their inner ecosystems are more different than alike.

The Human Microbiome Project sequenced the gut bacteria of hundreds of healthy adults and found exactly this. There was no single "healthy" composition. Some guts were dominated by Bacteroides. Others ran heavy on Prevotella. Some had thriving Faecalibacterium prausnitzii (one of the most important anti-inflammatory bacteria in the gut). Others had almost none and were still perfectly healthy.

The researchers’ conclusion: a healthy microbiome isn’t about having specific bacteria. It’s about what those bacteria can do, their functional capacity. Two completely different communities can perform the same jobs, like making short-chain fatty acids or synthesising B vitamins.

So when a probiotic brand says its formula "restores gut health," ask the obvious question. Restores it to what? There’s no universal template to restore.

You’ve probably lived this. A friend swears by a probiotic. Fixed their bloating, lifted their energy, cleared their skin. You try the same one and feel nothing. Or worse.

That gap is biology, not placebo.

In 2018, researchers at the Weizmann Institute gave the same 11-strain probiotic to healthy volunteers, then biopsied their intestines to see what actually happened. Two groups emerged. "Persisters," whose guts let the new bacteria settle in. "Resisters," whose existing microbiome kicked the newcomers out.

The split wasn’t random. It came down to each person’s existing bacteria. If your resident microbes already fill the niches the probiotic strains want, the newcomers have nowhere to go. They pass straight through.

So your friend’s experience tells you almost nothing about yours. Their gut had room. Yours might not.

What shapes your unique microbiome

Your gut bacteria didn’t assemble at random. A lifetime of factors shaped them, many specific to you.

Birth mode matters. Vaginal delivery seeds a baby’s gut with Lactobacillus from the birth canal. C-section babies first pick up skin bacteria like Staphylococcus. Those early differences can last years.

Feeding matters too. Breast milk carries human milk oligosaccharides (HMOs) that specifically feed Bifidobacteria, which is why breastfed babies tend to run much higher in Bifidobacterium.

Then geography and diet. The Indian gut differs from Western ones. Work from CSIR-IGIB and other Indian groups shows Indian microbiomes tend to carry more Prevotella (tied to plant-based, carb-rich diets) and different enterotype patterns than American or European ones. That tracks. Our plates are built on dal, rice, roti, and seasonal vegetables, not the meat-heavy Western diet most probiotic research rests on.

Antibiotics reshape everything. A single course of broad-spectrum antibiotics can cut gut diversity by 30% or more, and some species take months or years to return, if they return at all. Your lifetime of antibiotic use has left a mark no one else shares.

Stress, sleep, medications, even the water you drink, all carve out an ecosystem that’s distinctly yours.

The formula problem

Now look at what a mass-market probiotic tries to do. Take 3-5 strains (usually Western-researched ones like Lactobacillus acidophilus and Bifidobacterium lactis), put them in a capsule at a standard dose, and sell it to everyone. A 22-year-old student in Pune. A 55-year-old diabetic uncle in Chennai.

The student might have a Prevotella-rich gut with high diversity and an intact mucus barrier. The uncle might have lower diversity from years of metformin, less Akkermansia, and a very different metabolic environment in his colon. These two need completely different interventions. The same capsule makes about as much sense as the same prescription glasses.

The industry knows this. The science has been clear for years. But making personalised products is expensive and hard. It’s far easier, and more profitable, to sell one formula with vague claims about "digestive wellness" and "immune support."

Now look at what a mass-market probiotic tries to do.

The future of gut health isn’t a better universal formula. It’s dropping the universal approach altogether.

Your microbiome is a connected ecosystem. To intervene well, you need to know what’s already there, what’s missing, what’s overgrowing, and which strains and substrates would actually shift things in the right direction for your body.
